• Notice of Responsibility Form filled out by you and the landlord 

Are you moving? Here is what we need to end service in your name:

• Renter Moving Out Form filled out (renters only)
• Property Transfer Affidavit (property owners only)

Are you watering your lawn or filling your pool? Here is what we need to adjust your quarterly bill:

• Sewer Adjustment Form filled out

Are you unable to pay your Utility Bill before the shut-off date? Here is what we need to avoid shut-off:

• Authorization to Put Utility Bill on Taxes (property owners only)

All forms and documents can be mailed, delivered, or placed in one of our three (3) drop box locations at: Corunna City Hall, 402 N.
